What to Bring
- An up-to-date list of your medications (including vitamin supplements).
- An up-to-date list of your medical conditions and allergies.
- Information about your health care providers, including all of your doctors and their phone numbers (including previous dentist).
- Information about your emergency contacts.
- Your dental insurance or medical cards.
- Your dentures or partials…even if you don’t wear them.
Insured patients must have all necessary information for their dentist appointment. That way we can request benefit information from your insurance carrier. This includes your insurance card and/or the requested insurance information on the insurance section of the New Patient Form.
Please arrive 15 minutes early for paperwork. This will aid us in seeing you on time for your dentist appointment.
